Our statewide network is made up entirely of mission-driven, nonprofit community health organizations that provide essential care every day. These providers expand access to primary care by removing barriers like cost, lack of insurance, distance, and language — delivering critical services to the patients and communities who need them most. In doing so, they generate lasting value for the healthcare system and the country as a whole.

Driving Financial Sustainability with Value-Based Care
As an accountable care organization (ACO), we manage the administrative complexities of value-based care contracts, including performance tracking and network-wide goal setting, and provide the support needed to help our members achieve those goals. By participating together as a network, our members are incentivized to reduce costs and improve outcomes. We distribute the earned shared savings back to our members to diversify revenue and enhance patient and care team satisfaction, while reinvesting the remainder to sustain and grow the network’s long-term impact and financial stability.
