Over the past five years, our network has made meaningful progress in improving care for chronic conditions and mental health. These outcomes reflect our commitment to whole-person, data-informed care.
Diabetes remains one of the most common and costly chronic diseases in the United States. Affecting over 38 million adults nationally, nearly 1 in 4 people are unaware they have diabetes. Another 97 million have prediabetes, placing them at high risk. And the financial toll is steep, costing $413 billion annually and accounting for 1 in every 4 healthcare dollars.
Early detection, consistent monitoring, and team-based support can dramatically change outcomes. Over the past five years, our network has improved diabetes control by 9.5%.

Impact in Action: Whole-Person Support for Diabetes
A patient with uncontrolled diabetes wasn’t ready to start medication. Her care team began by listening and worked with her to create a plan aligned with her goals.
Through a partnership with a local nonprofit, she received diabetic‑friendly meals at home and ongoing support from a Chronic Care Coordinator. With this whole‑person approach, her A1c improved without medication, showing how trust, partnership, and coordinated care can move health forward.
Often called the “silent killer,” high blood pressure increases the risk of heart disease, stroke, and kidney failure. Effective management depends on ongoing, coordinated care and patient trust. Carrying an annual cost of $131 billion, high blood pressure disproportionately affects people of color, rural populations, and those with lower incomes.

Impact in Action: Supporting Blood Pressure Control and Access to Surgery
A patient needed orthopedic surgery but could not be medically cleared due to high blood pressure. His care team provided a home blood pressure cuff and support to help him track readings and make lifestyle changes. As his blood pressure came under control, he was cleared for surgery and successfully completed the procedure, improving both his mobility and quality of life.
Mental health is essential to whole-person care. In Colorado, many people experience symptoms of depression but are unable to access timely support due to cost, stigma, or system barriers.
By integrating depression screening and follow-up into primary care, our network has strengthened access to timely support.

Connecting Patients to the Right Care, at the Right Time
When timely primary care isn’t accessible, the emergency department can feel like the only option. Carina partners with community health organizations to strengthen access to trusted, local care and reduce avoidable emergency visits.
Since 2019, this work has contributed to a 26.2% decrease in emergency department visits across the network.

We support members in lowering the cost of delivering care by supporting integrated, team‑based primary care, improving coordination, strengthening prevention, and promoting efficient use of resources. These efforts help care teams thrive and supporting the long‑term sustainability of community health organizations.
When lower costs are achieved while meeting quality goals, the network is rewarded through it’s value-based care contracts, with shared savings reinvested across our network to strengthen access to care and support continued improvement.
