Connecting Care Across Colorado

Patients across Colorado often face barriers navigating complex and disconnected healthcare systems. We work with community health centers, rural health centers, critical access hospitals, mental health centers, State Medicaid, and other community entities who are mission-driven and provide high-quality, compassionate care, but rising demand and limited resources make coordination challenging.
