Health Center Controlled Network (HCCN) in Action

May 14, 2026

As a federally funded Health Center Controlled Network (HCCN), Carina Health Network supports Colorado Community Health Centers (CHCs) through shared health IT infrastructure, data analytics, quality improvement, and technical assistance services designed to strengthen value-based care and improve patient outcomes across the network.

Carina remains actively engaged in the national HCCN collaborative, where CEO Jason Greer serves as chair. Through this leadership, the collaborative advances a national data strategy focused on interoperability, shared learning, and innovation to better support CHCs nationwide.

Carina continues to provide ongoing technical assistance to Colorado CHCs, including support for:

  • Quality reporting and eCQMs
  • UDS reporting preparation
  • Workflow optimization
  • Population health analytics

This support includes one-on-one guidance, reporting validation, educational webinars, and collaborative peer-learning opportunities to strengthen data accuracy and performance improvement efforts.

Carina also convenes IT and operational network leaders through the Health Technology Network (HTN) and Data Security Committee (DSC) to address key topics such as health IT strategy, cybersecurity, compliance, interoperability, and data protection.
